What I'd like to know is why the TCAS system didn't prevent this crash.

The Black Hawk is fitted standard with the necessary transponder for TCAS to work.
And its mandatory for every commercial aviation aircraft in the U.S. to have one.

Actually, it does look like controller error or incompetence. Also, DEI, not competence, has been the priority at the FAA. Thousands of qualified, trained applicants with training were turned away. But their replacements made some really awesome Tiktok dance videos I'm sure. DEI may ultimately not be involved in this particular case, but there has been a growing number of "incidents."
