I know enough though about the basic scam - it is best in places with WEAK narrow network HMOs/Medicare advance but more PPO dominated markets. Usually it isn't fake patients - it is over billing Medicare for proceedures that were never done.

California actually has a TON of HMOs/Narrow network. So while we probably have the largest $$ in fraud (1/6thg of the country) on a per capita likely no.

I spent my career in healthcare finance. I'm curious about why you reached that conclusion. Not challenging it, just want to know your thinking. In my experience there are a variety of ways to scam Medicare. Some are more suited to Medicare Advantage (diagnosis mining or upcoding). Others, such as billing for unnecessary services, to traditional Medicare. And as far as I know the scam where someone gets hold of an individual's Medicare number and bills for non existent procedures or equipment works for both programs.

The exercise of attributing the bad faith vs ignorance part is not just some kind of an empty putdown, it is based on the fact that you really are not arguing in good faith, and this is a prime example.

Most people understand that when oligarchs acquire interests in the media, it is a strategic investment meant to increase their political influence and power, as opposed to a purely monetary decision based on the asset itself and its proforma.

Perhaps you are not fully aware of this, in which case your response could be attributed to ignorance, though I suspect you are not that dense and are mostly arguing out of bad faith here.

I thought that enrollees in Narrow network HMOs generate revenue on a a capitated/quasi-capitated basis.

I thought that enrollees in Narrow network HMOs generate revenue on a a capitated/quasi-capitated basis.

It is, with adjustments for age, sex and diagnosis. There has been a big issue with insurane companies, particularly United Healthcare, sending out nurses to find diagnoses that increase capitation payments. A bad approach in my opinion. It would be better to allow these increases only if the member was actually treated for the diagnosed condition.

A big factor in California is the presence of Kaiser. The Bay Area, when I reviewed the data around 2000 had the lowest Medicare cost (adjusted for local inflation) per member in the country. If Kaiser is scamming they're not very good at it
