The only voter fraud that I've heard about in recent years in my part of the world was an election official in a rural Washington county that was caught going through the ballots. When they found a race that had no vote cast, they filled in the bubble for the Republican candidate. Hard to detect unless you catch them in the act, which happened in this case.
