I'd guess it's done occasionally, but not as obviously as Green Bay did it. To make it less obvious, all you have to do is call a defensive play that is poorly suited to stop a run between the tackles.

And the offense has to really want to score, like when the game is tied as in that Den-GB Super Bowl. If the offense is already leading by a fair margin, they might do what Ott did and stop short of the goal line.

Yes, though you see it all the time that players forget the game situation and just focus on gaining as much as possible. Ott didn't.
