I think it absolutely can make a difference that large if (and I am not saying I know this is true) only people with symptoms are being tested.
However, I think you are right and this is not the case here - at least in the one state I chose to look at which is SD:
In SD the population is 884,659. This is about the size of SF whose population is 883,305.
SF has been seeing about 100 new cases each day. With a 5% positivity rate that means about 2000 tests are administered each day. SD is seeing 1300 new cases each day. To have a positivity rate of 57% that means about 2300 tests are administered each day in SD so people in both places are getting tested about as often.
