Correct, in every other wealthy country the government guarantees and subsidizes basic levels of coverage well above what we have in the US. I think the US should be doing this too and would be very much willing to pay taxes for that.
I think most people have no idea how much taxes would have to be increased to expand Medicare to everyone. Medicare has an unfunded liability of roughly $50 trillion. That's right, trillion. And because Medicare and Medicaid pay less than cost, there's an implicit tax embedded in private insurance premiums. That would have to be explicit. As someone said, 'regulatory capture'. Speaking as someone who ran a hospital-physician joint venture that provided service to Medicare beneficiaries, there is tremendous waste in Medicare, not to mention outright fraud. Providing 'free' healthcare through Medicare expansion would be a fiscal disaster. You'd have to change the model to direct governmental provision of care.

And as to health outcomes, once you factor out drug use, obesity and violent crime, the comparison isn't so rosy.

I don't think it has to be literally Medicare for everyone (though I know that's a popular campaign slogan on the left). More likely some kind of public-private hybrid would probably work best. I'm thinking something like they have in Switzerland or Japan.

I eyeballed the healthcare cost by year graph and I don't see much of a slow down, except for the absolute decrease during COVID, which of course has a significant impact on the trend line. I don't think that can be attributed to Obamacare.

I've read numerous studies asserting savings under Medicare for all, including one you referenced, and they all assume massive savings from reducing admin costs and eliminating profit. Typical figures cited are 12% admin and profit for private insurance vs 5% admin for Medicare. 7% of healthcare costs is a huge number and drives the savings assumed in these studies. These figures are superficially correct but in my opinion extremely misleading, for several reasons. (This is a pet peeve of mine.)

First, the figures are denominated by the 'cost of care', total spending on the provision of health care. In my view, a much more appropriate measure would be per capita spending. Because the cost of care for Medicare beneficiaries is 3 to 4 times that of private insurance, a per capita measure puts Medicare's admin burden right up there with private insurance.

Second, as far as I can tell the 5% figure only represents the CMS budge and doesn't include any part of other HHS spending. Some significant portion of the remaining HHS budget has to be related to Medicare.

Third, a significant amount of Medicare admin costs are included in other parts of the federal budget. Revenue (Medicare taxes) are administered by the IRS and Part B premiums collected through the Social Security program.

Fourth, (and this is minor but was always irritating to me) Medicare foists costs on to providers that private insurance doesn't. For example during my tenures as a hospital CFO the second highest paid employee in finance was the Medicare reimbursement specialist.

Finally, and probably most importantly, you're not going to achieve the cost levels of other countries unless you reduce the remuneration of physicians, nurses and other healthcare professionals by 20%, because that's the current gap in pay compared to other countries. As an example of how hard I think that would be, for years current legislation calls for a cap on Medicare physician fee increases. Every year congress overrides that provision.

Having said all that, I'm no fan of the current system. I'd like to see a lot of changes. Premium support. The ability to offer plans across state lines. Allowing non-employee groups (UC Alumni for example) to sponsor plans. A move to a system like Kaiser, that eliminates fee for service in favor of per capita payments.

Given the way we handle things, I don't see any change coming until we're up against a mandatory reduction in benefits when the Medicare Trust Fund is depleted.

Why do we screw up the handoffs in Olympics after Olympics?!?

These guys have mostly all been running relays since 9th grade or so, then they're together prior to the Games with professional coaching. Why is it so hard for us? I'm sure there must be something tricky about it that I don't understand, but it seems like it's always us!

Reminds me of Wilcox and special teams.

One missed baton pass is an accident, but several decades of failure is a clear pattern. Most of these sprint runners are individualistic prima donnas who lack the discipline to train for and execute a team relay competition.
